Dartmouth-Hitchcock Kendal Clinic – Hanover, NH

The DH-Kendal Clinic located within the Continuing Care Retirement Community of Kendal at Hanover is seeking an enthusiastic and experienced APRN or PA to join our team. Our DH-Kendal Clinic provides a broad spectrum of geriatric primary care and chronic disease management. Routine care is provided within clinic visits, independent living home visits, and assisted living Health Center visits. Acute issues are also addressed urgently within these settings. Emphasis is on wellness and palliative care for this unique and high functioning Kendal community of ~400 residents aged 65 and older. The clinic provider team includes one physician and two nurse practitioners. The ideal candidate will work intensively within an interdisciplinary team that includes nursing, social work, rehab, nutrition, and spiritual care.

Working within a team is a large component of this role so excellent and professional communication skills, with both patients and DH specialists, are critically important. Autonomous clinical skill is important for periods of working alone. On-call responsibilities are included in this role.

Qualifications

  • Graduate of an accredited APRN or PA academic program
  • National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA)

Required Licensure/Certifications

  • Eligible for RN licensure in the State of NH (APRN only)
  • Eligible for APRN or PA licensure in the State of NH
  • Eligible for Federal DEA Registration
  • BLS certification
